Request a Medical Report

Ministry of National Guard Health Affairs (MNGHA)

An electronic service that allows patients at one of the Ministry of National Guard Health Affairs (MNGHA) facilities or affiliated health centers for requesting and print available medical reports, such as sick leave report, burial permits, patient escort reports, and maternity leave certificates, without the need to visit MNGHA premises.

Log-in to the Patient Services portal using your Nafath, biometric, or credentials.
Go to the Main Page in the Patient Services Portal.
Click on the medical report icon.
Select a medical report request.
Select the type of report and visit for which you want to request a report for.

An active medical file that is eligible to get the service in one of the Ministry of National Guard Health Affairs facilities or National Guard health centers.
The existence of a previous visit to one of the Ministry of National Guard Health Affairs facilities or National Guard health centers.
